There are infections, and there are "holy mother of God, what the heck is that?"
This falls into the latter category, easily.
First, there's no way anyone should be awake during this procedure. Just from sheer fear of seeing what's being pulled from their lip and completing wigging out on everyone in the operating room.
Secondly, how does a worm even get stuck inside a lip? What entry point in the body leads to a space inside? That thing had to be implanted there, right?
Third, why is the doctor shoving the worm back in her face. "Oh my God, look what we pulled from your lip, sweetie." Refer back to the first point if you're confused as to why that's such a bad thing.
