Pato O'Ward hit the brakes and attempted to steer away from the wreck, but his No. 5 Chevrolet spun and backed into Rossi's car before bouncing into the air.
Romain Grosjean also spun and slammed into the outside wall as he tried to avoid the incident. His No. 18 Honda then came to a rest on the apron of the track with a small fire under the rear.
All three drivers were transported to the infield care center after the hard crashes. They underwent evaluation. Both O'Ward and Grosjean were evaluated and released.
However, IndyCar released a statement from medical director Dr. Julia Vaizer saying that Rossi was "awake and alert, and in good spirits." IndyCar noted that he remained in the care center undergoing evaluation.
The series then released a statement at 3:01 p.m. ET saying that "Alexander Rossi is being transported to a local hospital for further evaluation, per Dr. Julia Vaizer, IndyCar medical director."
Rossi, who drives for Ed Carpenter Racing (ECR), had closed out the weekend with his best qualifying effort at the 2.5-mile oval. He had posted a four-lap average of 231.990 mph, which locked up a spot on the front row between pole-winner Alex Palou and David Malukas.
This qualifying effort put the ECR driver in a position to challenge for his second Indianapolis 500 win and his first since 2016.
"It was a huge team effort," Rossi said after qualifying. "You know these conditions were really hard today. It was moving for every run. We didn't do anything the same for each of the three runs. Our last run was the best.
"That's a huge win for the 20 Java House guys, and a big win for Chevy to be on the front row. The car has been incredible all month. Obviously, you hope for pole, but a front row is so cool here and something I haven't accomplished for a really long time. Thrilled with today and how it went and can't wait to get on track tomorrow and make it better."
