The 86-year-old revealed Wednesday that doctors discovered "some abnormalities" during a recent series of scans. He is scheduled to undergo a biopsy on Friday.
As he always does, Vitale shared exactly how it feels.
"Like countless others, each time I go for any kind of test I am a nervous wreck," he said in a statement released by ESPN. "The anxiousness you feel about the uncertainty is off the charts."
He added that while he feels strong physically and mentally, the latest tests, including a PET scan, CAT scan and MRIs, showed areas of concern that now require further evaluation.
"They are recommending some additional testing to determine next steps," Vitale said. "I'm hoping for the best."

It's been a long fight.
Vitale has been battling cancer since 2021, including melanoma and lymphoma, followed by vocal cord cancer in 2023. He also underwent surgery in 2024 to address lymph node cancer.
Still, he keeps showing up.
Last month, Vitale made an emotional return to the broadcast booth, calling a First Four game alongside Charles Barkley. For many fans, it was a powerful reminder of just how much he's overcome.
Support poured in again.
"Our teammate. Our friend. We love you. In your corner, always," ESPN shared publicly.
Vitale has been a fixture in college basketball for decades. He joined ESPN in its early days and became one of the sport's most beloved voices, eventually earning induction into the Naismith Memorial Basketball Hall of Fame in 2008.
Through it all, his mindset remains the same.
"As always," Vitale said, "I plan to remain as active as ever and fight as hard as I can."
That's who Dickie V has always been.
