The 34-year-old made the announcement during an emotional interview with Nine's 100% Footy, describing the interview as something he had long feared.
"I was living in denial," Evans said. "From a young age I knew that I'm gay, but I went down every other avenue to build up these walls to be someone to escape who I am."
He added: "I still can't believe I'm here talking to you. This is definitely my worst nightmare, but I know if I surrender, it's the start to a new life. I've been fighting a war within since I was about 15 years old, and it's not sustainable."
Evans also spoke candidly about the personal struggles he faced while keeping his sexuality private, saying there were times he was homeless, battling drug use and questioning whether he wanted to keep living.
He said those experiences ultimately convinced him to share his story in hopes of helping others.
"I know there are people struggling with the same struggle I've gone through," Evans said. "I'm very blessed that I can come here and talk to you and be able to save a life or two."
Evans also revealed that he had been subjected to blackmail over his sexuality in recent years.
"Now I've come and spoken about it, they've lost their power," he said. "Fear, shame and guilt — I've cut ties with all that. I feel peace within and I feel a weight has lifted off my shoulders."
The former professional rugby player said he was still in the process of having personal conversations with family members, including his parents, before the interview aired.
"I love my mum and dad so much," Evans said. "By the time this airs I would have told them already. I can't wait for that moment because then I'll be free."
Evans played in the National Rugby League for several clubs during his professional career, including the Sydney Roosters, Parramatta Eels and New Zealand Warriors.
