According to an arrest report that TMZ obtained, Woods claimed distraction as a factor in the crash, telling officers he was looking at his phone and changing the radio when traffic slowed in front of him.

However, deputies say Woods showed "severe signs of impairment" at the scene.

The report notes he was sweating heavily, moving slowly, and appeared lethargic. His eyes were described as bloodshot and glassy, with extremely dilated pupils. At the same time, officers said he was "extremely alert" during parts of the investigation.

Woods had two hydrocodone pills in his pocket at the time of the crash that led to his arrest in Florida. The pills, marked "M367," are a prescription opioid used to treat pain.

Woods told deputies he had taken prescription medication earlier in the day but denied drinking alcohol. A breathalyzer later showed 0.00. He refused a urine test.

The collision caused his vehicle to flip after hitting a truck and trailer. Damage was estimated around $5,000. Witnesses helped Woods out of the car.

He agreed to field sobriety tests but struggled physically, citing multiple surgeries and visible injuries.

Woods was charged with DUI with property damage and refusal to submit to a lawful test. He's due in court next month.
